One of my favorite go-to op-amps is the LMC6482/6484. I think it would meet
your specs although it is not likely the least expensive one that does. It
is CMOS-based so the input bias current is phenomenally low - but you do
pay for that in terms of cost. Output is true rail-to-rail if the current
is low enough.
