OK, tracing the pinout from the SIL connector for the PIC KIT 3 to the ICD chip on the header, I found the Vpp pin was not making it through. So, I ordered a new AC164110 RJ-11 to ICSP adapter which includes the PCB and a short RJ cable. Swapping stuff around, the problem turned out to be the RJ cable. Sometimes I spend more time debugging my tools than I spend debugging my project! Anyway, I'll get back to the project tomorrow!
