I never used it but I remember hearing a talk at the IEEE Cherry Hill
test conference by a guy who built a hardware fault simulator for the
RCA 1802 for NASA-JPL.

As I recall the 1802 was the first CMOS micro and you could get it
built/qualified for space use (ceramic packaging, Silicon on Sapphire,
etc.).

Anyway, I remember in the Q&A someone asked him something to the
effect of: "why bother with hardware fault simulation?"  his response
was:

"our customer returns come in the form of fireballs"
