Malcolm Gladwell's book Outliers has a chapter about airplane crashes=20 due to cultural norms/conventions that is fascinating. It was almost=20 always a combination of bad weather, technical malfunction in the plane=20 or at the airport, and communication between pilot/copilot and air=20 traffic control.
