Indeed, coding reference data for database systems can lead to such
things; probably caused by semi-automatic processing of injury causes,
and if there are multiple turtle strikes it gets coded.  We have
similar in our wonderful systems downunder.

(Today, I spent a few hours teaching school children how to use
TurtleArt, a Logo like programming language, hooked up via Bluetooth
to a SparkFun RedBot.  There were no injuries.)
