Hey Neil! I know you said U.S., but I can personally recommend Djula @ Datatehnik in Serbia. For small quantity runs, I believe people often just use Priority Mail International flat rate boxes to send them parts. Quality is good, he's been patient with me :) , knows his stuff, and his prices are very, very good -- even for manual through-hole where you can't wave solder. Our components are everything from 0805 passives through to tiny BGA CSPs, DFNs, battery holders, and TO-39 cans. Just tossing it out as an alternative.
