It looks like it might be me - and I apologize profusely to anyone affected. However I didn't do anything specifically to initiate the request / application and would be really, really interested to know how it happened. At worst I didn't do anything to inhibit the request, but then I didn't know I needed to. So many apologies Carl (and anyone else involved), and please let this be a warning to everyone else.
