I bought an Obihai OBI110 VOIP adapter for about $50 online and use it with Google Voice, for free. I live in the US and pay $1.50 per month to Anveo.com for 911 emergency service. The call quality is great 95% of the time and I can even FAX with it. I have a cable modem with 15Mbps down / 1Mbps up. I am not sure how well it would work on DSL.
