You'll have to consider the regulator as a circuit on its own that
needs current to operate, no matter what current is passing to
the load. The regulator will be more or less efficient for various
input-output differentials but Iq is always additional to Iload. A
modern SMPS IC generally has a much lower Iq because the
output current is supplied on demand by a digital circuit
