Need more info- what are you planning on dumping this signal into? Are you making a PCB or do you need a breakout board for the jack? Sparkfun (disclaimer: my employer) sells a nice 3.5mm to .1" header breakout board (http://www.sparkfun.com/products/10588) along with the jack itself.
