While much more often then not the issues you see are due to either not properly understanding the models (which I believe is the case this time), or not properly modelling the circuit (resulting in failed sims), there are times the problems are due to the tools or models themselves. I've seen SPICE models that were just wrong, I've seen ones with VERY border case type issues.
