It could be as simple as a single word getting trashed. To start with, I woul= d read back the data in the PIC with a programmer and compare the result to the HEX file, or do the verify directly with the programmer if it has that capability. It might be easier to guess what happened when you know what got trashed. Since this code contains a bootloader, there are instructions in there somewhere that erase and write program memory. One possibility is that you have a bug that causes erroneous jumping, sometimes into this code. Also, = I have not generally been impressed with Microchip firmware. Just because it came from Microchip doesn't mean it's beyond suspicion.
