I've been quite happy with PCBCART (http://www.pcbcart.com/), in China.

They have asked sensible questions, been responsive and flexible.  And I'm
pleased with the quality of the boards they've made for me.

I've used them for the PCBs for the kits I sell through my web site, e.g.
the Christmas Star is a PCB cut into a 5-point star, but although this is
"non-standard", PCBcart didn't charge me anything extra for that.
