I usually use Gold Phoenix for prototypes.  $109 delivered to North America
for however many boards fit in 100 square inches, solder mask top and
bottom, silkscreen on top, and electrically tested.  No limit on holes, and
specified hole size is the final hole size.  Usually about 1 1/2 weeks from
when I order over the web to when boards show up.
