I had a similar problem a couple of times also, so no you're not crazy :-) I 
did the same thing, started a new project etc. Never worked out what caused 
it either. I also reinstalled MPLAB, and (touch wood) it hasn't happened 
again since. On the programming/debugging front, I'd go with Xiaofan on 
getting an ICD3, I took his advice a while back and I'm very happy with it.
