The information they wanted was way outside my "personal privacy level". So, I faked it all, put clearly wrong information in every single box. It was "sent for approval", and was approved a few hours later. I doubt a person read any of it since it was clearly bogus information I was supplying. I don't know what's gotten in to them, but to require that sort of personal information is unreasonable to me.
