This probably relates to particle size being a nice match for certain lung structures - which is what makes the wrong sort of asbestos* and Berylium so nasty. The diesel particles concerned can be filtered but it's hard and thus expensive to do so (very small size). * Asbestos breathing harzard varies extremely widely with type, but this tends not to get noticed when people are decrying it.
