I am such a goose and felt the need to tell someone, everyone.

For sometime now I have occasionally had issues where running notepad2
and Firefox gave the appearance of refusing to restore or maximise,
just showing up in the taskbar.  I tried uninstalling, registry
hacking, googling. The only fix has been to re-install OS.  (I tend to
do this often)

Finally (just now) it occurred to me that I also occasionally connect
an additional monitor and suspected that they had set themselves to
default to the second screen. I just reconnected my second monitor and
there it was, notepad2 smiling smugly back at me.

I can now move on.
