The register will reflect the state the pin should be in, high or low. However, external circumstances may force the pin to a different state. For example, there might be a difference between the two if you set a pin to a high output state but externally it is directly shorted to ground. The register will show the pin is trying to drive high but because of the short to ground the pin may actually be in a low state.
