when your SX-Key seems to work from time to time, but is not found otherwise, it is most likely that it is not defective. Very often, a weak power supply is the reason for problems like this. The SX-Key (serial version) is powered from the target system via the Vdd and Vss header pins, and "solid", stable 5 Volts are expected here. Often, a 10µF tantalum cap attached across the Vdd/Vss pins of the target system's SX-Key 4-pin header helps a lot.
