I've had a similar problem. I'm still working through some of the details. My programmer is clone PICKit2 I got off E-Bay. By tying pin 4 MCLR to Vdd with a pullup resistor and within my programmer turning off the MCLR option it programs and works. It's still a bit intermittent but before I couldn't get it to do anything. But since I didn't have a 10k resistor I'm not so sure that it might not improve when I do. I don't know if it's a combination of having config option for MCLR wrong or what. I should find out later tomorrow after I get to the store to get some 10k resistors.
