Thanks for all the replies. This is definitely a tall barrier to
self-employment.

Worst case, I think we can use the state sponsored insurance for my wife and
have a separate policy for the rest of us. Not ideal, since that insurance
isn't quite as good, but it is a solution.

We are continuing to look for something better.
