Practical for a hobbyist to build or practical for a well funded spy 
agency to build or somewhere in between?

I think with a sufficiant development budget (doing stuff small always 
drives up costs, thinner PCB traces, tiny hard to handle packages etc) 
you could get one down to wristwatch size without too much trouble. Any 
smaller than that and I think you would run into problems with finding 
small enough batteries speakers and microphones.
