In the case of software -- the bugs happening over and over again (same bugs, different decade... literally) is crazy. "Oh look, yet another engineer that thinks the network/backplane/real world will always deliver his/her packets un-mangled!" :-) Of course, then they just blame it on the hardware engineer, and move on...
