Try to use a regulated supply for starters and pull MCLR up to Vdd with 10kOhms.
A regulated supply would be a 5V1 zener across Vdd and Vss and Vdd supplied from
the wall wart through a 50 ohm 1W resistor. Don't forget decoupling at the chip
(10uF should be enough).
