Hi Ray,
I don't have access to any of my headers or notes right now but I do 
have a question for you.  Are you using the MCLR line as I/O or as 
MCLR?  I seem to recall that it needs to be MCLR and pulled up when 
using the ICD-2.  I'll check into it further tonight but I'm getting 
slammed at work right now.
