Regardless of what triggers it, are you doing everything "by the 
book"?

All input pins terminated high or low.
All non terminated pins as outputs.
Watchdog either disabled or disabled.
No pin voltages > Vdd or < Vss by even a smidgeon EVER.


There are others, but any of these may kill you after an indeterminate 
period of time or activity.
