There was definitely a bug in the ProProg firmware which has now been fixed. Go to http://www.embedinc.com/picprg/sw.htm to install the latest software, which includes the PPRG28.HEX file in the SRC > PICPRG directory, then use that to update your ProProg firmware. I can program your board reliably with your HEX file with a ProProg running the PPRG28 firmware. I'm still not comfortable with the 10nF capacitor on Vpp, but it works with it there, at least with my setup.
