I checked and I actually send two files.  The first is the Excellon 
"tool rack" file that specifies the sizes of the drills used.  The 
second file specifies the locations of all the holes.

For a test board with one "hole" and one plated hole/pad of the same 
diameter, there's no differentiation in the Excellon files.  One tool 
shown with two locations.
