Chris found the answer. Looks like capacitance was the problem. I built a compact test protoboard and ran it at various voltages and debug worked with them all.

Thank you to everyone who offered suggestions, I really appreciated it.

And, a big thanks to Chris, I don't think I would have solved it without your help.
