How are non-plated though holes specified in gerber files?  I'm using Eagle, 
btw, and I notice that on locations with a hole (rather than a pad) some 
board houses thru-plate them and others don't.

Is there a formal spec/definition for this in the gerber file?  (This would be 
preferred over putting notes in the readme, as I've been noticing that most 
board-houses don't pay attention to the readme files.)
