Regarding the altitude of Flight 93 (for cell phone usage), and answering my own question: This page presents the flight recorder data for 93. Most of the flight was at 35,000 feet; then in 20 minutes it descended approximately linearly from 40,000 to 5,000 feet, which was the time of the last call.
