I just confirmed with mine, with MPLAB v7.41, it said 01.00.04.00 for
debug executive.

Just to confirm, as someone already pointed out in recent message, did
you pull up GPIO3 to Vdd?  I means when the header is in your target
circuit.  And if you use only the header alone, I think you might not
get anything out of it since GPIO3 is not pulled to Vdd.
