It requires a bit of both... proper documentation needs at least a certain understanding of what the target audience needs. I agree that documentation is a discipline that seems to be underrated. I don't know whether there are any reliable stats about this, but I think the number of worthwhile projects that failed because of insufficient documentation is probably considerable. You know what I mean when you've seen how all of a sudden everything /flows/ because everybody has a good doc that tells them in their language what exactly they have to do to make it work.
