Note that that's 0.1" pitch - the pads reduce that markedly. This sort of voltage is routinely handled by TO220 parts. As voltages get up people have been known to "stagger" the leads to achieve greater PCB spacings. Then the package pin to pin spacings become the limiting distance. This is usually only a problem where there is contamination.
