If you have LVP enabled in your CONFIG, RB5/PGM
*have* to be low to not enter LVP mode.

And you are *STILL* mixing up what is reasonable to do
at *RESET* (that is, set all pins as inputs), and what whould
be done in a particular *DESIGN* with "unused" pins (that is
set them as output *or* add pull up/down resistors).
