I just put the circle on the dimension layer.  Getting it cut
out is going to be more dependent on the PCB fabricator than on
the cad package.  In my case, the LPKF software can route around
a particular layer or trace, so all I need is SOMETHING that shows
up in the design files I import.  I would think the milling layer
would NOT show up in the standard exported files, and therefore
would not be such a good idea.  The "obvious board outline" that
shows up normally on the silkscreen layer is PROBABLY enough.

(I have not sent any boards to real PCB vendors...)
