Getting it cut out is going to be more dependent on the PCB fabricator than on the cad package. In my case, the LPKF software can route around a particular layer or trace, so all I need is SOMETHING that shows up in the design files I import. I would think the milling layer would NOT show up in the standard exported files, and therefore would not be such a good idea. The "obvious board outline" that shows up normally on the silkscreen layer is PROBABLY enough. (I have not sent any boards to real PCB vendors...)
