If and sometime are unavoidable. ie we have already had several existence proofs which show it can and does happen - we have (probably) contained the ones we know about. In each case the events were unexpected and unpredictable AND used systems and techniques essentially indistinguishable to those which we are setting loose in the wild to do their own "experiments". I didn't define "utter catastrophe". I'll accept any normal meaning of that. eg Hurricane xxx caused an utter catastrophe ... .
