I've seen that happen when the chips are programmed using the PICStart Plus. Not because of the PICstart but because of the ZIF socket (so, no ICSP). I think the error was because a bad contact, because I saw it on some chips that where not perfectly parallel to the socket (ZIP socket)... Any chances its something like that?
