Of particular note -- that our engineer thinks may be causing your troubles -- is the exclustion of the 1.5K resistor that connects between pin 7 (USBDP) and pin 5 (RSTOUT#) on the FTDI chip.  There are a few other things that we noticed, but JB doesn't think that they are the cause of your problems.
