I had pretty much the same requirements a while ago. I ended up using something open-source called BugManager, by Clarisys. I don't remember what their distribution options were; I got the Delphi source and have hacked it a fair amount since. So I don't have an installer for either the original or my hacks, and I don't remember if you can get a precompiled version... But, FWIW, it's worked well for me as a solution for that same problem.
