At what point in the process do you "pull the plug"? Before the program cycle or between program and verify? I believe you're using an ICD2, but what's it connected to? (ie. what is the PIC plugged in to?) The problem is less likely to be the ICD2 itself, more likely to be at the other end of the grey cable.
