IIRC all the devices that had LVP seemed to have problems with the early
versions. It was almost as though there was no lockout of the LVP pin when
using HVP, so you had to hold the LVP enable pin in the program mode state
to allow HVP. I suspect the problem is probably fixed for all current
production, which may be why the ICD2 doesn't worry about connecting the LVP
pin.
