Do you have access to Adobe Photoshop? Just open the PDF page in one of those programs (select the desired page and bitmap resolution), change to RGB color if it was in CMYK, then save as BMP. You can get pretty much as high resolution on the bitmap as you like (provided you have the memory to handle huge bitmaps), *IF* the original PDF had vector Postscript stuff to describe the tracks. If it's a GIF or JPG image internally, there will be other issues. (or use Ghostscript/GSView, as Bob Blick suggests- download both programs eg. http://www.cs.wisc.edu/~ghost/gsview/get46.htm , and use the "Convert" function).
