While the RJ12 connector is handy, having only one tiny, flimsy wire to make a GND connection has turned out to be a problem. MUCH more reliable if you take the shoeshine-wax case apart, locate GND, and connect an external GND clip of #22 hi-flex wire to your prototype PCB. Also the published connections for the ICD2 RJ12 are incomprehensable. To clearly understand it, you will need to locate which wire is GND and work out in both directions from there.
