We use FrameMaker (from adobe, these days, I think) for our manuals.
It was supposed to be the internal documentation standard for things
like specifications and design documents as well, but apparently it
was SO painful for most people to use that most of those end up being
done in Word or (like me) plain ascii.

I think a lot of people seriously overestimate how "pretty" their
manuals "must" be; I'll trade pretty for a good index any day...
