Martin, You only need to reserve RB6 & RB7 (PGC/PGD) & MCLR for the ICD2. RB5 is sometimes tied low to inhibit LVP, but in my experience this is not strictly necessary, and the pin can be used as an IO pin. RB3 is not used by the ICD2.
