Now one step up: remove the max232, connect the cable to the progger, short the max pins that connect to the proggers connector, and again verify that an echo exists (and no echo with the short removed, did you check that too?). Next: max232 in place, apply 5V, remove 16F628, short the PICs UART pins, check again for echo.
