Thanks to you all,

The problem was not the offsetting of the code but the bootloader Microchip
offers. It is not for the PIC18Fxxx but for PIC18F452 and it does not write
correctly the Flash of the PIC18F442. I'm have my hands on it now.

Simply add the switch -A200 to the linker command line.
