Dick Smith Electronics in Oz/NZ happen to have the Piher C-11 rotary 
encoder with integral push button - they are selling them off and they 
have them at a ridiculous price.

Rapid in the UK did have some, but they have n horrendous penalty for 
non EU orders

Also Digikey used to have the Piher ones
